Purpose of the FormThe Citizen Report Form is used to document incidents where there is no crime scene evidence or suspect information. Citizens complete these report forms primarily for:
- Reporting property loss to insurance companies
- Documentation of child custody agreement violations
- Civil disagreements (where no crime occurred)
- Minor accident on private property involving property damage only
Obtaining a Report FormCitizens may fill out our online form, download the form from this site, obtain them from the police department, or they may be issued to citizens by officers in the field.
Once the Citizen Report has been completed, it must be turned into the police department, where a case number will be assigned.
