Citizen Report

Purpose of the Form


The Citizen Report Form is used to document incidents where there is no crime scene evidence or suspect information. Citizens complete these report forms primarily for:
  • Reporting property loss to insurance companies
  • Documentation of child custody agreement violations
  • Civil disagreements (where no crime occurred)
  • Minor accident on private property involving property damage only

Obtaining a Report Form


Citizens may fill out our online form, download the form from this site, obtain them from the police department, or they may be issued to citizens by officers in the field.

Once the Citizen Report has been completed, it must be turned into the police department, where a case number will be assigned.

ADA and Section 504


Notice Under the Americans with Disabilities Act

In accordance with the requirements of Title II of the Americans with Disabilities Act ("ADA") and Section 504 of the Rehabilitation Act, the City of Kingsburg will not discriminate against qualified individuals with disabilities on the basis of disability in admission or access to its services, programs, or activities including federally assisted services, programs, or activities.

Additionally, the City of Kingsburg does not discriminate on the basis of disability in its hiring or employment practices and complies with all regulations promulgated by the U.S. Equal Employment Opportunity Commission under Title I of the ADA.

Effective Communication: The City of Kingsburg will generally, upon request, provide appropriate aids and services leading to effective communication for qualified persons with disabilities so they can participate equally in the City of Kingsburg's programs, services, and activities, including qualified sign language interpreters, documents in Braille, and other ways of making information and communications accessible to people who have speech, hearing, or vision impairments.

Modifications to Policies and Procedures: The City of Kingsburg will make all reasonable modifications to policies and programs to ensure that people with disabilities have an equal opportunity to enjoy all of its programs, services, and activities. For example, individuals with service animals are welcomed in City of Kingsburg offices, even where pets are generally prohibited.

Anyone who requires an auxiliary aid or service for effective communication, or a modification of policies or procedures to participate in a program, service, or activity of the City of Kingsburg, may contact the City department that is sponsoring the event or the City’s ADA/Section 504 Coordinator. The request should be made as soon as possible, but no later than 48 hours before the scheduled event. Anyone requiring an auxiliary aid or service for a City Council Meeting should contact the City Clerk’s Office at 559-897-5821 ext. 1104 as soon as possible, but no later than 48 hours before the start of the meeting.

The ADA does not require the City of Kingsburg to take any action that would fundamentally alter the nature of its programs or services, or impose an undue financial or administrative burden.

The City of Kingsburg will not charge an individual with a disability or any group of individuals with disabilities to cover the cost of providing auxiliary aids/services or reasonable modifications of policy such as retrieving items from locations that are open to the public, but are not accessible to persons who use wheelchairs.

How Do I File a Grievance?

Any individual who believes he or she has been discriminated against on the basis of disability, in the provision of services, activities, programs, or benefits by the City of Kingsburg can file a grievance in accordance with the grievance procedures below or contact the City's ADA/Section 504 Coordinator.

ADA Meeting Accommodations

Arrangements for a sign language interpreter, documents in Braille and other auxiliary aids and services necessary for effective communication for qualified persons with disabilities need to be made as soon as possible, but no later than 48 hours before the scheduled event.

Please contact City Clerk's office at 559-897-5821 ext. 1140 if an auxiliary aid is needed for a City Council Meeting. For all other public meetings, contact the City department sponsoring the meeting.
